Boston Bone & Joint Institute assists patients with completing medical leave, disability and other employment-related paperwork. A processing fee applies to all requests, including revisions and extensions to previously completed forms.

How to Submit a Request
To ensure your paperwork is processed as efficiently as possible, please follow all four steps below.

Click the Request Paperwork button on this page to access our secure, self-service portal. You may also scan the QR code below to complete your request on your mobile device.

Complete all required fields and submit your information. You will receive an email confirming your initial submission.
Important: Your request is not complete at this stage. You must also select the specific paperwork you need and complete payment. -
Choose the appropriate paperwork or form revision and add it to your cart.

Proceed to checkout and submit payment. You will receive a second email confirming that your payment has been processed.

Processing Times
Please allow the following processing times:

Standard Processing: 7–10 business days

Expedited Processing: 4 business days
Processing times begin only after we have received your completed request, all required supporting documentation and payment.
If additional information is needed, a member of our team will contact you.
