Immigration Physical Exams (USCIS Form I-693)

BBJI OccMed has a designated United States Citizenship and Immigration Services Civil Surgeon on staff to complete Form I-693 for immigration medical examinations.

Our office will complete the required evaluation, prepare the I-693 form, and provide you with the sealed original documents for submission to USCIS. Lab results, a copy of the completed form, and sealed documentation are typically available within one week.

​

If required vaccinations are missing, we can provide immunizations in our office.

What to Expect

There is no need to fast before your immigration physical. Please eat and drink normally prior to your appointment.

​

Your visit will include:

​

  • An interview with the Civil Surgeon to review your medical history and vaccination record

  • A brief physical examination

  • Blood tests and urine collection as required by USCIS guidelines

 

The exam fee includes:

​

  • Tuberculosis blood testing (IGRA) for individuals age five and older

 

Additional testing, such as syphilis (RPR) blood testing or urine testing for gonorrhea and chlamydia, will be performed if required under current CDC and USCIS guidelines.

All examinations are conducted in accordance with CDC Technical Instructions for Civil Surgeons.

What to Bring
  • USCIS notice or instructions, if available

  • Vaccination or immunization records

  • Government-issued photo identification (passport or U.S. driver’s license)

  • Credit or debit card for payment

 

For children, vaccine requirement is based on age and typically are the same as those required by most schools in the US. For those 18 years and older, typical vaccine requirements include:

  • Polio vaccine documentation at age four or older

  • Current-season influenza vaccine if exam is during flu season

  • Tdap or Td within the past 10 years

  • MMR (two documented doses or positive titers for those born after 1956)

  • Varicella (two doses, positive titer, or documented history of disease)

  • Hepatitis B (three documented doses or positive antibody titer)

 

Please bring government-issued photo identification (passport or U.S. driver’s license) Credit or debit card payment preferred. We cannot accept insurance for immigration physical exams. An itemized receipt will be provided for potential reimbursement.

Cost

Immigration Physical Exam: $485

​

The fee includes the required examination and standard laboratory testing. Additional vaccines or laboratory tests, if needed, are billed separately and reviewed with you during the visit.
